    • All opt-out requests will be reviewed within 7-14 business days. Additional verification may be required.

    • The request may be denied if sufficient proof of authority is not provided.

  3. Privacy & Data Protection

    • Personal information submitted in this form will be used solely for processing the opt-out request.

    • Any approved removal will be applied only to the public Memorial Wall and does not affect internal records.

  4. Finality of Decision

    • Once a request is approved and processed, the deceased’s name will be removed from the Memorial Wall.

    • If denied, the requester will be notified with an explanation and may appeal the decision within 7 days.

  5. Legal Compliance
