Please be advised that this system is an inurnment consultation (call) appointment to finalize your inurnment form. The date you will pick is NOT the date for the actual inurnment at St. Thérèse Columbarium.

  • Registered Death Certificate
  • Cremation Certificate
  • Exhumation Permit*
  • Right of Perpetual Use (Title) or
  • OR of Vault's Full Payment
  • Valid ID of Vault Owner
  • Letter of Authorization*
  • Valid ID of Representative*
